Fees & Forms

The standard hourly fee is based on a 50 minute session: 

  • Individual - $90
  • Couple - $135
  • Group/Family - Standard individual fee + $20/person

Acacia Counseling does not take insurance however your insurance may reimburse some expenses on an out-of-network basis.

Payment is due at the time of service and can be made by cash, check or credit card.

Cancellation can be a reality in times of necessity or emergency. Acacia Counseling requires a 24 hour notification. Cancellations 24 hours prior to your scheduled appointment will result in a waived fee. Any cancellation within 24 hours of your scheduled appointment will require payment for the full session fee.

If you're a new client, please complete the following forms and bring them to your first therapy session.

If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information:
